Risk Management

St Andrew’s Toowoomba Hospital is committed to establishing an organisational culture that ensures risk management is an integral part of organisational objectives, strategic plans and all management systems.

St Andrew’s Toowoomba Hospital has a focus on providing a systems approach where the systems in place are considered rather than the act or omission of individual employees. A reporting culture is encouraged at all levels of staff, with a ‘no-blame’ or 'just' response to all reporting. Employees must feel free to report all hazards, near misses and minor incidents without fear or punishment.

The risk management process includes:

Identification of substantial risks
  • Assessment and prioritisation of those risks
  • Identification of appropriate actions to manage risk; and
  • Implementation of the action and evaluation of their effectiveness
  • Maintenance of a Risk Register and Risk Register Action Plan detailing controls in place

St Andrew's maintains a risk register which is a register to maintain documentation of risks, identification of the level of the risk, risk prioritisation and planning of action to be taken.

The risk register addresses risks in the following areas:
  • Clinical Risk: Risks associated with patient safety management.
  • Financial Risk: Risks that would have a negative financial impact on the hospital.
  • Facilities Risk: Risks associated with emergency procedures management such as fire and safety or security.
  • Human Resource Management Risk: Risks associated with staff performance and skills.
  • Operations Risk: Risks associated with hospital reputation, i.e. complaints management, media management, customer service.
  • Information Systems Risk: Risks associated with the Information Technology systems and privacy.
  • Workplace Health & Safety Risk: Risks associated with staff safety.
  • Environmental Risk: Risks related to the external environment.
